Send consultation forms before a visit

Consultation forms are part of the Forms and reviews app. Turn it on under Settings → Apps if you don't see Catalogue → Consultation forms in the menu.

Make a form

1. 1Go to Catalogue → Consultation forms and press + New form.
2. 2To save time, pick one under Start from a ready-made form, such as Hair colour consultation or Skin (patch) test record, and change it to suit you.
3. 3Give it a Form name and add your questions. Answer types include Short answer, Yes / no, Pick one, Date and Tick to agree.
4. 4Choose When it's sent (see below) and press Save form.

When it's sent

• Send with booking confirmations: the form's link goes in the confirmation email of every new booking, online or from the calendar. Untick it to only send the form by hand.
• For these services: tick services to send it only for those. Tick none to send it with every booking.
• Only once per client: once a client has filled it in, new bookings don't send it again.
• Ask for a signature: the client signs with their finger or mouse and types their full name.

Send one by hand

Open the client (or the appointment), go to Forms, choose the form and press Send a form. It's emailed to them. If they have no email, open it from the Forms tab and copy the link, or hand them your tablet to fill it in at the salon.

See the answers

Filled-in forms show on the client's Forms tab and on the appointment, with the signature. Recently filled in on the Consultation forms page shows the latest.

Switch off stops a form being sent but keeps every answer. Changes to a form apply to forms not filled in yet.
